Type of Plan

PPO using Blue Cross/Blue Shield national PPO network.


All clergy* under full-time appointment, including full-time local pastors serving a conference church are required to enroll in the plan. Dependents may be included to age 26.

  • Part-time clergy/laity are not eligible.
  • Laity employed by the conference, church or agency of the conference, working full-time at least 1,560 hours per year (30 hours/week) may enroll. Dependents may be included to age 26. Laity enrollment must meet the 75% requirement. See 2017 Guidelines.
  • Full time clergy appointed to local church: Premiums are the responsibility of the local church.
  • Clergy not under appointment to a local church: Call Veronika Varga, Benefits Assistant 732-359-1036 for information.
  • Laity: Premiums are the responsibility of the insured, payable through the employer.


Premium is a blended premium for all plan types: In 2019: $15,000 annual/$1,250.00.


Annual comprehensive vision exam using Vision Service Plan participating physician. Vision Hardware Discount Program is also available through VSP.

MDLIVE – new!

MDLIVE is a telemedicine service available for HealthFlex participants designed to handle common non-emergency conditions such as cold/flu, sore throat, rash and more – after normal office hours, when traveling, or any time you cannot get in to see your regular provider.

  • State-licensed, board-certified doctors (including pediatricians) via phone, secure video or an easy-to-use MDLIVE mobile app can diagnose your symptoms, prescribe non-narcotic medication, and send prescriptions to your pharmacy of choice.

Are included at no additional cost to the participant in the Plan. Many are associated with cash incentives.

Massage Benefits are available within the HealthFlex plan. 

Participants currently enrolled in HealthFlex have benefits which include massage therapy, acupuncture and naprapathy. Coverage for massage therapy, acupuncture and naprapathy is limited to 35 combined visits per calendar year. The plan pays 50% of the billed amount.

You are unable to use your insurance card at time of service, therefore, you must pay for your services in full and then submit for reimbursement. For benefits to be paid, you must obtain the following information from the provider of services and include when submitting for reimbursement:

  • Claim form
  • Itemized bill, including name of provider, address, Tax ID or SSN,
  • Date of Service, CPT code or description of service rendered and total charge.
  • Massage therapist must be licensed in the state where the service is performed.
