Interpretation Equipment Request Form
Please note that all requests are pending approval. You will be notified once your information has been reviewed and if your request has been accepted or declined. Please contact us if you have any questions in the meantime.
The interpreter must complete interpretation device training at the Mission and Resource Center before equipment will be signed out for all requests.
A deposit of $100 by credit card will be due at the time of equipment pick up.
