Pastoral Support Forms

Note about file formats and downloading the forms on this page:

The forms in the Church Conference and Pastoral Support packages are offered in both PDF (Portable Document Format) and Microsoft Word or Microsoft Excel (when applicable).

The Microsoft Word or Excel version of these forms will allow you to complete them on-line, and then save the completed form to your computer. To save the form to your computer, click on File, then Save As, then name the document and indicate where you want it saved. 

While Microsoft Word and Excel are widely used programs, not everyone has them in their computer.  For those without Microsoft Word and Excel, the PDF version of the forms should be used.   The PDF format files allow all users, regardless of their individual computer?s operating system and/or word processing program, to open and complete forms with the free Adobe Reader program.  The PDF form will allow you to print the document to give to the District Superintendent and have a paper copy for your files.  The PDF documents will NOT allow you to save the filled-out form to your computer.

Please note that the some forms are available ONLY as PDF files.  They are the UMPIP Before-Tax After-Tax Form, Billing Change Form-UMPIP, Employment Eligibility Verification I-9 Form, Accessibility Audit Form, Annual Deacon Report, Lay Speakers Annual Report Form, Higher Educational Biennial Review and Renewal Form, and the Higher Educational Diaconal Ministry Annual Report are only available in PDF format as they are produced by outside agencies.
